About the role


We are currently recruiting for a compassionate and motivated Clinical Case Manager on a permanent full time basis at join our South Australia team. This is an opportunity to be an integral part of developing RetireAustralia to become one of the leaders in the industry, placing our residents at the centre of everything we do. As a passionate leader, you will mentor and inspire your team to achieve the purpose of integrating care into the service we provide and being part of a dynamic care team.


You will be responsible for supporting the growth and development of our Home Care program providing case management to our residents that supports their independence through flexible service delivery. Building and maintaining a relationship with our residents and their families is vital. Through your compassion and understanding of the industry, you will be able to actively listen to our residents and their perspectives to build these relationships. This will allow you to maintain a caseload using a holistic approach to meet the specialised needs of our residents. 


About you

You will be passionate about the retirement industry and contributing to the lives of others. It is essential to have a background in home care service delivery with demonstrated experience in case management or care coordination. You must have strong communication skills and the ability to build rapport with a diverse range of people.

  • Case Management qualifications (highly advantageous) 
  • RN or EN background 
  • A valid National Police Check 
  • A current First Aid Certificate with current CPR
