About the role

 We are currently recruiting for a compassionate and motivated Case Manager to our team at The Verge, which is located alongside the Burleigh Golf Course. This is an opportunity to be an integral part of developing RetireAustralia to become one of the leaders in the industry, placing our residents at the centre of everything we do. As a passionate clinician and care educator, you will provide general advice and education in order to assist our residents and their families in understanding and navigating MyAgedCare, as it relates to Home Care.

 You will be responsible for supporting the growth and development of our Home Care program at The Verge, including assisting residents to understand and navigate MyAgedCare (MAC), supporting them and/or their families with aged care assessments and providing advice and assistance to source appropriate providers for Government funded Home Care. The role will also entail the coordination and set-up of non-complex fee for service (FFS) care, provided by RetireAustralia Care staff, during business hours only.

 Building and maintaining trust and rapport with our residents and their families is vital. Through your compassion and understanding of the industry, you will be able to actively listen to our residents and their perspectives, which in turn will create strong connections with those people receiving care services from various providers. This will also ensure RetireAustralia is well positioned to take on its own home care package (HCP) caseload when the timing is right.

 We are an organisation that is open to and encourages family friendly working arrangements. This part-time role has the flexibility to operate during school hours, or other hours that might be suitable for the successful candidate.

 About you

You will be passionate about the retirement industry and contributing to the lives of others. It is essential to have a background in home care service delivery with demonstrated experience in case management or care coordination. You must have strong communication skills and the ability to build rapport with a diverse range of people.

  • Case Management qualifications (highly advantageous)
  • RN or EN background
  • A valid National Police Check
  • A current First Aid Certificate with current CPR
